Care Management is the process used to ensure that Food Pantry clients, as well as those seeking financial assistance, receive one-on-one consultations with a trained Care Manager.
Care Managers are volunteers who have received training on how to assess the financial, physical, and emotional needs of clients. They understand and know how to direct clients in accessing the various local, county and state resources that are available. Referrals to Chesapeake Church resources such as the Chesapeake Christian Counseling Center and Benevolence Assistance (financial help with electric bills, medicines, etc.) are only provided upon assessment by a Care Manager.
Care Managers have the privilege of working with clients at a deeper level and relationship; building trust, providing emotional support, encouragement and prayer support. In one-on-one sessions, Care Managers work with clients to establish long and short term goals that will assist them in moving towards self-sufficiency.
